In the State of Colorado, each region is designated as a Regional Accountable Entity (RAE). Each region has a specific organization that supports its RAE. The two RAEs we work with are Colorado Access (RAE 4) and the Colorado Community Health Alliance (RAE 3).

• Colorado Access / CHP+ (RAE 4) serves the following counties: Adams, Arapahoe, Denver, Douglas

• Colorado Community Health Alliance (CCHA) (RAE 3) serves the following counties: Boulder, Broomfield, Clear Creek, El Paso, Gilpin, Jefferson, Park, Teller

If you currently reside in a county that is not included as part of RAEs 3 & 4 (see image below), we unfortunately are unable to provide services to you at this time. We encourage you to check this website to search for in-network providers.
